Should absolutely have to pay higher health insurance rates. Don't know why this is so taboo.
The car insurance industry does this to young drivers as they are higher risk. Should be the same here.

Additionally, they should be required to wear some type of odometer device that can demonstrate they have exercised prior to being allowed entrance into restaurants or really any public venue. Kind of like a Fat Card/Passport.
